How we build a care plan with your family
It starts with a complimentary, in-home consultation. We come to the house rather than asking you to describe it over the phone, because the house tells us things a phone call can't — where the steps are, how far the bathroom is from the bed, whether the kitchen is being used.
- You reach out. Submit the form and one of our local care coordinators will contact you within 24 hours to discuss your family's needs.
- We visit. We sit down with your loved one and whoever else in the family wants to be there. We ask about routines, medications, mobility, meals, and what a good day looks like.
- We draft the plan. Which services, which days, how many hours, and who is responsible for what. In plain language, not jargon.
- We start, then we adjust. Needs change after a hospital stay, after a fall, as dementia progresses. The plan is meant to be revisited, not filed away.
