What an aging-at-home plan actually is
An aging-at-home plan is a structured, proactive plan that puts companion care, personal assistance and safety checks in place before a crisis occurs. Instead of scrambling after a fall or an emergency room visit, you already know who is coming, on what days, and what they are watching for.
Building one starts with a complimentary, in-home consultation. A care coordinator visits, meets your loved one, looks at the house, listens to what the family is already doing, and then puts a customized care plan on paper. There is no charge for that visit and no obligation attached to it.
