Most families call us somewhere in the middle of a hard week. A parent has had a fall, or a hospital discharge date has landed sooner than expected, or the daily phone calls have started to feel like they aren't enough anymore. You don't need to have the answers figured out before you reach out. That's what the assessment is for.
The in-home consultation is complimentaryand it happens where the care would actually happen — at home, in the rooms your loved one moves through every day. When you get in touch, one of our local care coordinators will contact you within 24 hours to talk through what's going on and set a time to come out.
What happens during a free in-home assessment
A coordinator sits down with you and, where possible, with your loved one. We listen first. Then we walk the home together, because the layout tells us things a phone call never will — where the trip hazards are, whether the bathroom works for someone unsteady on their feet, how far it is from the bed to the kitchen at two in the morning.
- A conversation about daily routines: mornings, meals, medications, sleep, and what's become difficult
- A look through the home for clutter, lighting and mobility concerns that raise the risk of a fall
- Questions about memory and orientation, if cognitive changes are part of the picture
- Appointments, errands and transportation needs, including any upcoming hospital follow-ups
- What family members are currently covering, and where the gaps and the burnout are
- A customized care plan: which services, how many hours, which days, and what we watch for
There's no obligation attached to the visit. Some families book care starting that week. Others take the plan, sit with it, and call us back a month or two later when the time is right. Both are fine.

The plan we build draws from six services. Most families start with one or two and adjust as needs change.

- Respite & Recovery Care — relief for family caregivers, plus structured hospital-to-home recovery support to help prevent readmissions
- Transportation — hospital to home, home to doctors' appointments, and other care-related trips
- Cleaning Services — keeping the home clear of clutter and the injuries that come with it
- Travel Buddies — support before, during and after travel, including personal care and mobility assistance
